It features three hallmarks - one on the ring, one on the counter ring, and one on an arm of the cross. The Order of Queen Maria Cross, established by law no. 201 from 1917, was primarily intended "to reward those who served the country in wartime or during epidemics in peacetime, particularly in the sanitary field". The administration of the order was the responsibility of the Ministry of War, which also took care of the proposals for the awarding of the decorations, being approved by the king through this ministry.
